Chaperone 

Before completing the application below, ensure you have reviewed the chaperone description and requirements.

To volunteer you will be required to provide proof of Safe Environment Clearance from your parish. Documentation can be provided in the form below or at a later time via email (send to safetydirector@prolifedallas.org). Do not pay for camp until you have been formally accepted as a chaperone.
